The Hartford – Employer-Paid Basic Life and AD&D
Agencies are able to select the level of benefit they will provide to eligible employees.
Basic Life coverage available (Self): $10,000 to $200,000
Basic Dependent Spouse Life: $10,000 (optional, can be requested)
Basic Dependent Child(ren) Life: $5,000 (optional, can be requested)
The Hartford – Employee-Paid Supplemental Life
Available to employees and their eligible dependents through payroll deduction.
Supplemental Employee Life
Guaranteed Issue: Up to $200,000, in increments of $10,000, without evidence of insurability (EOI)
Maximum Issue Amount: Up to $500,000, in increments of $10,000; evidence of insurability (EOI) required for benefit amounts above $200,000
Supplemental Spouse Life
Guaranteed Issue: Up to $50,000, in increments of $5,000, without evidence of insurability (EOI)
Maximum Issue Amount: Up to $150,000, in increments of $5,000; evidence of insurability (EOI) required for benefit amounts above $50,000
Supplemental Dependent Child Life
Maximum Issue Amount: Up to $10,000, in increments of $2,000
